Left atrial appendage thrombus: Risk factor or source of cerebral embolism in patients with nonvalvular atrial fibrillation? (part 5)

The main limitation of this study is the subjectivity of diagnosis of LAAT, especially concerning small structures in the left atrial appendage. Although overestimation of the number of thrombi was possible, all studied groups should have been similarly overestimated. Moreover, high prevalence of LAAT in patients with atrial fibrillation and recent embolism found in other studies seems to concur with data from our subgroup with recent ischemic stroke. Despite the retrospective character of the study and monoplane evaluation of the left atrial appendage, we consider the presented data interesting mostly because none of the patients in any of the three groups studied with TEE was anticoagulated before or at the time of the examination. This was true despite obvious indications for such treatment in most cases. With current indications and practice of stroke prophylaxis, future comparative evaluations of TEE results in larger subgroups of nonanticoagulated patients with chronic atrial fibrillation are becoming unlikely. In summary, our data suggest that left atrial SEC is a persistent marker of increased stroke risk, while LAAT may be the source of individual embolic episodes.
